About this role

Eventus WholeHealth delivers an integrated model of care to adults in skilled nursing facilities, assisted living facilities, independent living, and personal homes. Services are provided through a network of providers including Physicians, Nurse Practitioners, Physician Assistants, Care Team Advocates, and in-house Support Staff. Telemedicine uses videoconferencing to send and receive patient medical information and pictures.

Telemedicine providers deliver medical services to patients at assigned contract sites according to site-specific protocols. Providers render medical consultations with family, facility staff, and specialty providers when necessary. Medical services are provided via telemedicine during 7 pm - 7 am shifts with some weekends.

Day-to-day involves providing primary-care telemedicine services consistent with family medicine, geriatric medicine, or internal medicine standards. Manage facility relationships, patient consents, CCM, and billing per Eventus protocols. Perform on-call tele triage, confirm consents, and follow telehealth visit etiquette.

Include family members in visits as appropriate and ensure proper equipment availability. Complete documentation and sign orders immediately after visits. Establish open communications with facility staff, providing instructions on medications and caregiver information.

Requirements

  • Active Nurse Practitioner or Physician Assistant license in NC, IN, or KY
  • Knowledge of standard of care for family medicine, geriatric medicine, or internal medicine
  • Familiar with telemedicine equipment, delivery platform, and software
  • Ability to render medical consultations with family, facility staff, and specialty providers
  • Experience managing narcotic prescriptions per Eventus RX Policy
  • Skilled in tele triage and following site-specific protocols
  • Competent in EMR documentation and notifying patient's regular provider of triage plan

Responsibilities

  • Provide primary-care telemedicine services in accordance with Eventus site-specific protocols
  • Provide on-call tele triage per established protocol policy
  • Confirm treatment and telehealth consents are signed prior to telemedicine visit
  • Follow Eventus telehealth visit etiquette protocols and complete documentation immediately after visits
  • Manage facility and other partners relationships, organization, patient consents, CCM and billing
  • Establish and maintain open and positive communications with facility staff and administration
  • Provide verbal and/or written instruction or feedback regarding medications and pertinent caregiver information
  • Adhere to Eventus standards, policies, and procedures and all regulatory agencies governing health care delivery
