Ecumen Hospice is hiring a Triage Nurse, RN for part-time telephone triage support in Assisted Living and Hospice. The role provides care that exceeds expectations for those in later life stages, honoring and supporting their significance. Join a mission focused on comfort and quality of life.

Independently deliver professional nursing services over the telephone, accurately documenting symptoms, assessments, advice, and responses. Follow policies, procedures, and protocols to ensure consistency, improve outcomes, and satisfy clients and staff. Follow up on higher-risk situations from family or staff.

Work with a leading nonprofit senior living provider, Minnesota’s 8th largest organization with over 160 years of experience. Schedule includes 16 hours bi-weekly from 3pm-11pm every other weekend, with opportunities to pick up additional shifts. Targeted pay range is $36.00-41.00 per hour based on experience and qualifications. Access benefits like paid Sick & Safe time and Employee Assistance Program.

Requirements

  • Licensed as a Registered Nurse in MN and maintain licensure or obtain within 60 days of hire
  • 1 year of care management experience in home care or hospice
  • Strong clinical and relational skills
  • Ability to analyze situation and implement solutions
  • Ability to handle multiple priorities, ambiguity, frequent change and high stress situations
  • Ability to communicate effectively in both verbal and written formats
  • Proficient with Microsoft Office Suite
  • Ability to follow all safety rules, regulations, policies and procedures of the facility

Responsibilities

  • Independently provide professional nursing services over the telephone for patient and resident care
  • Accurately document symptoms/condition, assessment, advice provided, and patient/caller response
  • Follow policies, procedures, protocols to ensure consistency and improve outcomes
  • Follow up on situations representing higher risk or need from family or staff
  • Analyze data, trends, and develop solutions to improve services and outcomes
  • Provide high level nursing support for patient care questions at assigned site, including incident follow-up
  • Efficiently and accurately document in multiple electronic health records
  • Maintain high level of standards in clinical skills and professional ethics
