About this role

The Care Nurse Supervisor (CNS) manages the successful operation of the Resident Care Department under the leadership of the Resident Care Director (RCD). Responsible for oversight, coaching, and direct supervision of all resident care service providers in the community. Management scope includes direct supervision of resident care positions, resident assessment, and management as defined by the RCD.

Identifies and assesses the clinical status of potential residents and obtains medical information release from primary care physician as needed. Develops a plan of care in collaboration with the RCD based on identified physical and psychological problems, including goals and interventions. Coordinates care needs with community providers via effective case management and measures vital signs per state regulations.

Directs and supervises daily work assignments of Resident Care Assistants (RCAs), serving as first point of contact for questions or concerns. Provides input into performance reviews and disciplinary actions, and participates in hiring and termination processes for resident care associates. Ensures compliance with resident care regulations, infection prevention, and personal safety procedures.

Conducts SAMM/MAR and RCA documentation audits bi-monthly, assessments per Benchmark standards, and facilitates weekly resident tracking meetings in absence of RCD. Observes and reports significant changes in resident health to RCD, physician, and family per state regulations. Actively participates in the QA process and responds to emergencies.

Requirements

  • Licensed Practical Nurse (LPN) within scope of practice
  • Expertise in clinical skills and nursing judgement
  • Knowledge of state regulations for resident care and medication management
  • Ability to assess resident risk and conduct assessments/reassessments
  • Experience in developing service plans and case management
  • Proficiency in vital signs measurement and health documentation
  • Supervisory skills for directing care staff
