About this role
BAYADA at Bayhealth seeks a Hybrid Admissions/Case Manager Registered Nurse for full-time or part-time opportunities with adult patients in their homes. This role services clients throughout Kent County, DE, concentrated in Smyrna, Clayton, and Dover areas. You will provide skilled nursing and rehabilitative care as an integral multi-disciplinary team member.
Make home visits to clients in designated geographic territories, following a designated care plan per patient's needs. Perform duties including medication administration, wound care, treatments, and procedures. Monitor conditions, report changes to Clinical or Client Services Manager, and execute doctors' orders.

Requirements
- Current license as a Registered Nurse in Delaware
- Minimum of one year recent verifiable clinical (medical/surgical) experience
- Prior home care experience strongly preferred
- Graduation from an accredited and approved nursing program
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills with prior electronic medical records (EMR) experience preferred
- Ability to travel to cases as assigned
Responsibilities
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions and report changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Provide case management and coordination
- Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet
