The Hospice Triage RN receives phone calls from hospice patients and families. They assess needs of the patient and family to coordinate care and schedule home visits as needed. This role manages phone-based triage in a remote setting.

Collaborate with transdisciplinary team members and facilitate communication among the team, Hospice, and the patient’s Attending Physician. Work Tuesday through Saturday from 8:30am to 5:00pm. Coordinate with clinicians for timely home visits.

Requirements

  • California RN license
  • CHPN certification, preferred
  • Minimum of three (3) years of experience as a triage/telephonic nurse within the last five (5) years
  • OR one (1) year of RN experience in hospice or palliative care
  • Experience assessing patient needs via telephone in hospice settings
  • Knowledge of pain and symptom management pursuant to physicians’ orders
  • Ability to coordinate clinician scheduling for home visits

Responsibilities

  • Receive phone calls from hospice patients and families
  • Assess needs of patients and families to coordinate needed care over the phone and schedule visits as needed
  • Manage patients’ pain and symptoms pursuant to physicians’ orders
  • Obtain changes in orders as needed and update the patient’s plan of care and medication list accordingly
  • Assess patient and family needs for home visits and contact appropriate clinicians to schedule
  • Notify patient and family of expected home visit times
  • Collaborate with transdisciplinary team members
  • Facilitate communication among transdisciplinary team, Hospice, and patient’s Attending Physician
