Chestnut seeks nurses to drive integrated care across primary care and psychiatric programs.

This candidate will be based in our Granite City Health Center but may occasionally support Maryville or Belleville. The schedule for this role is Monday-Friday, typically 8 am to 5 pm. Anticipated starting pay for new hires is between $19.85 - $40.84 per hour.

Carry out health care plans designed by practitioners while administering treatments, medications, and medical tests. Assess patients for acute physical issues and gather patient health histories. Act as a liaison among prescribers, patients, laboratories, PCPs, other providers, pharmacies, and staff to ensure continuity of care.

Provide health teaching as well as support and guidance for patients in maintaining a therapeutic environment. Manage referrals to specialists or facilities by assessing needs and ensure smooth communication between patients, providers, and payers. Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness.

Requirements

  • High school diploma or equivalent
  • Practical nurse licensed in Illinois or registered nurse licensed in Illinois
  • Basic computer skills and experience including MS Word, email, videoconferencing, e-prescribing, and entry into an electronic medical record (EMR)
  • Ability to complete required substance-use training within first week of employment to be assigned to detoxification services
